- Estimated repair costs based on my worst-case experience: $8,000–15,000 over 3 years
3-year TCO: ~$48,000–$55,000 if nothing major breaks. But if it fails twice? Potentially higher than new.

Here’s the thing: I’m not a laser engineer. I can’t predict failure rates. But I can calculate the cost of missing that early-April event for Maria. The clinic estimated revenue from that single patient’s package (6 treatments + follow-ups) at $12,000. And Maria had already referred three friends. If the equipment wasn’t operational by mid-March, they’d lose more than just one patient.

The Turning Point: Why Certainty Won

I almost went with the refurb just to save $40k upfront. The Lesson: Time Is the Real Currency

In procurement, we obsess over dollar amounts. But sometimes the biggest cost isn’t in the PO—it’s the opportunity lost when you can’t deliver. When a patient walks in and asks “how long after Fraxel to see results?” the answer depends not just on the device’s specs, but on whether that device is actually operational when needed.

What most people don’t realize: the “standard turnaround” from a budget vendor often includes buffer time that they use to manage risk. But when you are the one absorbing that risk, the cost shifts to your clinic. The Solta Medical premium bought us something the refurb couldn’t: delivery certainty. In emergency situations, I’d argue that’s worth paying extra for.

Take this with a grain of salt—I’m a procurement guy, not a clinician. If you’re a doctor debating between devices, definitely consult a medical equipment specialist. But from a cost perspective, remember: a cheap device that fails is more expensive than a reliable one that works.
